I'm a little surprised about the Maestro's plpe organ. Wasn't that whole adventure a story written by one of their fans who thought he figured out the perfect way to kill the JLA? As such, the pipe organ shouldn't have existed. So did somebody get a hold of that story and try to use it in an untold tale?

Anyways, this relic from a nonexistent case could provide the basis for an Untold Tale of the JLA, just as you say. Perhaps, somehow that story was found by a criminal mastermind with a good deal of resources who decides to build the Maestro's pipe organ himself in an attempt to destroy the team.

Maybe there was a case with a villain who could make stories real, and he wanted to destroy the Justice League. He couldn't think of any villain from literature powerful enough to beat them, but had the idea to Make the Maestro real, since he nearly beat them in a fictional story.

Just a thought, but could someone using Doctor Destiny's Materioptikon manage to actualise potential or blueprinted artefacts? And if I recall correctly, that is one of the trophies. Now we just need to find a likely supervillain to discover its existence and hijack it. One problem- it wouldn't be the original mountain Secret Sanctuary, given its location was secret until Snapper's fall from grace. How impervious to detection was the satellite, though? Would it have an alien concealment device or forcescreen? Would its location be known to national space agencies, to avoid troublesome potential collisions from their exploratory probes or satellite altitudes? I can certainly imagine Lex and possibly other supervillain scientists hacking its location and getting into its guidance and domicile systems.
So...Lex and other tech supervillains have an untold adventure in which they actualise potential threats to try to best the League...?

Maybe someone was doing routine dusting and cleanup and accidentally turned the Materioptikon and Snapper was sitting in the Sanctuary daydreaming? And he daydreamed about the Maestro's adventure and materialized the pipe organ and everyone decided it was great to keep it as a trophy... the only question is, who would be so careless as to turn the Materioptikon on?
There might actually be a story in that idea. Suppose they put a new trophy near the Materioptikon that radiated enough power to cause the Materioptikon to function at very low power, and there was a plague of materialized annoyances in the Sanctuary as it responded to the thoughts of some of the heroes... and someone finally got fed up with all these seemingly random annoyances popping up and figured it out? Perhaps Ralph, it seems like his kind of mystery...

I can think of several ways the JLA might realise something's wrong- Superman's microscopic vision picks up irregularities, the Flash realises something's interfering with his superspeed control, the Atom detects some irregularities at the sub-atomic level within the satellite, Green Lantern's power ring could be used to detect its existence, and so on.

I was thinking more along the lines of a series of larger annoyances, for example, one the members, like J'onn, sitting in the meeting room thinking about a recent infestation of mice in his police station, and thinking he was glad they weren't Martian mice, and the Materioptikon creates a next of Martian mice, which probably breath fire as protection from the larger Martians, or the Flash recallling his battle with the Turtle, and the next time the JLA meets, they discover that the Sanctuary is infested with super strong baby turtles, things like that.
But the things you mention are equally likely, especially since we are writing comic books!

Super strong baby turtles with teensy-weensy swords? I was thinking they might be wearing tiny red capes with 'T' symbols
